256MB cards seen as 128MB are still seen as 128MB card (just tested on Peg2).
Oh, and don't try this on a G5 as it will(might) lead to a non booting system.
My guess is that it fails to add the RAM to system already maxed out on memory (got 2.5GB installed).
Might also be an issue on 2GB G4 PowerBooks and PMac.
