Does it happen when you boot from ISO/CD? Some weird keyb or kvm maybe? Sounds like something is borked quite much. Never had problems with project icons, even back in 1.4.5 times with nighly ambients.

Also, btw. inline rename in lister mode is fixed in 3.10. It didn't work correctly for quite some time.
