Have no idea where you find that bugger, but I would make a wild guess that it is no way a "standard" chip but something costum made for a specific project.
Maybe the IC orginally planned for the DIL-socket became unavailble during production and someone came up with this to avoid changing the PCB.
Or maybe manufactoring didn't allow for the 2 square chips to be mounted directly onto the PCB.
