No. There's been nothing extra to pay since 2.x was first released (1.x was free anyway).
The license is limited to your hardware though, so if you upgrade to a different machine in future then you'd have to pay for a new license for the new machine.
If your current machine breaks and you replace it you can transfer the licence from the old one for free though.
Most updates have introduced support for new machines, so MorphOS team gets new registrations from existing users upgrading - it's a fairly organic approach which doesn't leave any users stuck using old versions - a damn fine approach in my opinion.

Any bunring program will work, but make sure to "burn image to disk", not just create a disc with the ISO file on it (an easy mistake to make). If you're not sure, just check the disc contents afterwards - you should see C, Devs, Libs, S, etc. If you don't see these and just see morphos-3.x.iso on the disc then you've done it wrong.

Probably possible, but complicated. You'd have to put the ISO (not the files) onto the stick and pass a boot arguement pointing to the ISO as a boot source. HFS doesn't (afaik) support Amiga protection bits, so copying the files/directories onto USB is likely to result in FUBAR.

Hope you get your iBook set up successfully, just to reiterate I'd recommend keeping it simple to start with, and stick to the recommended method of burning a disc and booting.
